However, certain non-personal information is recorded by the standard operation of the Company’s internet servers. The Site’s various mailing lists, downloads, special offers, contests, registration forms, and surveys may request that you give us contact information such as your name, mailing and/or e-mail address, demographic information such as your age and gender, and personal preference information such as your preferred software and interests. I, the undersigned owner/handler (client), do hereby agree that Leigh Siegfried, Opportunity Barks Inc and all Opportunity Barks employees and agents will not be liable for any damage or loss, such as; damage from disease, death, running away, theft, fire, injury to or from persons, other dogs or property, or by any other causes resulting from the failure of the dog to respond to any cues or behaviors taught by Opportunity Barks Inc or resulting from counseling and advice supplied to the owners/handlers (client) of the dog.
I hereby guarantee that my dog is healthy and up to date on all legally required vaccines or titers. I understand that my dog’s behavior now and in the future is solely the responsibility of myself, the owners/handlers (client) of the dog.
General Policies: There is no one approach that works best for all dogs, therefore Opportunity Barks reserves the right to remove or refuse service to any client/dog. We also maintain the right to remove a dog from a group class setting and transfer any fees towards a private lesson program if we deem the class setting too stressful, overwhelming, or disruptive to any dogs/clients present.
All appointments are intended for the dog and primary handlers who originally book the session (this excludes other pet professionals including groomers, pet sitters, and walkers). Additional dogs who attend a session will be required to fill out all paperwork and pay additional fees upon completing the session.
Group classes require ALL participants to be over the age of 16. Opportunity Barks maintains the right to require any attendant under the age of 16 to remain outside of the training space.
Photo Policy: I agree to grant to Opportunity Barks Inc and its authorized representatives' permission to record on photographic film and/or video, pictures of my participation in any and all classes, workshops, and events. I further agree that any or all of the material photographed may be used, in any form, as part of any future publicity, illustration, advertising, or Web content used to promote Opportunity Barks Inc, and further that such use shall be without payment of fees, royalties, special credit or other compensation.
Cancellation and Rescheduling Policy: We sympathize with clients with emergency situations, but experience has demonstrated that exceptions to our policies are too costly for our business, disruptive to our schedules, problematic for the training process and unfair to other clients. Therefore, we maintain the right to charge you for your canceled appointment if it does not follow the guidelines below. Your requested appointment time and date is confirmed upon receipt of your payment.
A 72-hour cancellation policy applies to all scheduled appointments. Any appointment canceled (or rescheduled) with less than 72 hours notice, and any no-shows will be fully charged for the cost of the session. Our automated system, allows for reminders with the date/time listed, therefore showing up before/after the proper time will result in a "no-show" status.
Rescheduling of a class is subject to a $50 transfer fee. A $25 fee will be applied for returned checks as well as all associated bank fees.
Package Policy: Purchase of packages of any and all recurring appointments, workshops, events or socials are non-refundable and non-transferable. If you purchased a one on one training program, and you have not had an appointment within the last 4 months, we maintain the right to schedule you for a behavior evaluation before we offer follow-up lessons.
